ELIGIBILITY:
Most Medicaid eligibles are required to participate in CommunityCARE 2.0. For exceptions to this requirement contact the CommunityCARE 2.0 Program.
COVERED SERVICES:
CommunityCARE 2.0 enrollees are entitled to the same Medicaid covered services as those eligibles not in CommunityCARE 2.0.
Most Medicaid covered services must be provided by the PCP or must be authorized by the PCP prior to the service being rendered.
COMMENTS:
Effective January 1, 2011, the existing CommunityCARE Program was revised. The current version, CommunityCARE 2.0 is a system of comprehensive health care based on a primary care case management model. Medicaid eligibles are linked to a Primary Care Provider (PCP) responsible for coordinating health care services, either through direct service or appropriate referral authorization to specialists and/or ancillary providers.
