Welcome to the Commodity Supplemental Food Program (CSFP) Participant Preliminary Certification Webpage

The U.S Department of Agriculture prohibits discrimination against its customers, employees, and applicants for employment on the bases of race, color, national origin, age, disability, sex, gender identity, religion, reprisal, and where applicable, political beliefs, marital status, familial or parental status, sexual orientation, or all or part of an individual's income is derived from any public assistance program, or protected genetic information in employment or in any program or activity conducted or funded by the Department. (Not all prohibited bases will apply to all programs and/or employment activities.)

How Do I Qualify for CSFP?

To participate in the CSFP program, applicants must reside in the state of Louisiana and meet CSFP income eligibility requirements established by the Federal Poverty Income Guidelines.

Gross Income Elderly (130% Over age 60)

How Can I Apply for CSFP? 

If you feel that you might qualify for the CSFP Program and would like to apply, please follow these steps.

       1.  Access, fill out and print the Preliminary Certification Form for CSFP. 

       2. Mail the completed application to the following address:

       3. Or you may fax your completed application to 504-241-3102


 I've Sent in My Form. What Now?

Once your CSFP Preliminary Certification Form has been received, the Food for Families organization will review your information to determine your eligibility. You will be notified within 10 business days of receipt of your completed form on whether you are eligible for the program.

You will also receive information on how to reach a local CSFP distribution center to complete your certification if you are eligible. You will need to bring additional information with you to finish signing up for CSFP. Find out what additional information you will need here.

Please note that placement on the program is dependent on availability of caseload space or slots. When slots are unavailable, you may be placed on a waiting list and later certified as slots become available on a first-come, first-served basis. You will be notified within 10 business days of receipt of your completed form if you are to be put on a waiting list for the program.
