My Place LouisianaMy Place Louisiana is the state's program for the federal Centers for Medicare and Medicaid Services Money Follows the Person (MFP) Rebalancing Demonstration, designed to help states try new ways of delivering Medicaid services. The demonstration program is designed to help people to move, or "transition" from an institution into home- and community-based living settings, such as a home or apartment. 

The Louisiana Medicaid Office is working with the Office for Citizens with Developmental Disabilities (OCDD) and the Office of Aging and Adult Services (OAAS) to implement My Place Louisiana, which is  a four-year (2008-2016) project focusing on Medicaid funding and following participants in transition from nursing facilities, hospitals and ICFs/DD to home and community-based living services. 

My Place Louisiana Serves

  • People with developmental disabilities who are currently residing in an Intermediate Care Facility/Developmental Disabilities (ICF/DD): People who a developmental disability and currently live in a public or private ICF/DD (Intermediate Care Facility for Developmental Disabilities) are eligible to receive a Waiver through the My Place Louisiana program that enables them to move to a community living setting. 
  • Children or adults with developmental disabilities currently residing in a nursing facility, hospital or rehabilitation hospital: People who have a developmental disability and currently live in a nursing home are eligible to receive a Waiver through My Place Louisiana to move to a community living setting. 
  • People 65 years or older residing in a nursing facility, hospital, or rehabilitation hospital: People who have a developmental disability and currently live in a nursing home are eligible to receive a Waiver through My Place Louisiana to move to a community living setting.
